Developed following a cross-sector maternity safety roundtable organised by Fletchers Solicitors, this white paper explores a critical question: if we already understand many of the causes of avoidable harm, why are the same failures still happening?

Drawing on national reviews, NHS data and insights from clinicians, investigators, legal professionals and families, the report examines the gap between learning and implementation, and what is needed to turn lessons identified into lasting change for women, babies and families.
